Er lijkt sinds het aanbieden van het adviesrapport van de Gezondheidsraad aan de Tweede Kamer op 13 maart jl. niets gebeurd te zijn, maar niets is minder waar. Er is hard gewerkt achter de schermen.
Een van de leden van het Burgerinitiatief woonde de procedurevergadering van de VWS-kamercommissie op 29 maart jl. bij. Daarin werd besloten minister Bruins om advies te vragen. Wij kregen ook informatie dat dat advies niet bindend is voor de commissie.
Sinds de start van het Burgerinitiatief hebben we kunnen bouwen op en steun gekregen van vele internationale ME-experts met wie wij nauwe contacten onderhielden. In dit kader meldden wij al eerder (zie ons bericht van 11 mei 2018) dat 61 internationale experts na het verschijnen van het Gezondheidsraad-rapport een brief schreven aan Minister Bruno Bruins en de commissie VWS. In hun brief benadrukken de internationale experts wat de ziekte is en vooral niet is, en dringen zij aan op een substantiële en meerjarige investering in biomedisch onderzoek.
Wij stuurden die brief begin april met een begeleidend schrijven en verzoek tot overleg aan de Minister. Met als resultaat een uitnodiging voor een bijeenkomst met drie stafmedewerkers van Minister Bruno Bruins, waarbij wij als Groep ME Den Haag alsook de ME Vereniging, de ME/CVS Stichting, de ME/cvs Vereniging en de Steungroep aanwezig waren. Deze bijeenkomst vond afgelopen woensdag 11 juli plaats.
Doel van de bijeenkomst was om onze mening te vragen m.b.t. de aanbevelingen zoals geformuleerd in het Gezondheidsraad- rapport. Namens de Groep presenteerden mr. Arthur de Groot en Rob Wijbenga onze visie in het kort. Wij schreven daarnaast een concreet plan en boden dat samen met een geüpdatet versie van de brief van inmiddels meer dan 70 internationale experts aan tijdens het overleg. Tegelijkertijd is dhr. Bruins dezelfde informatie verstrekt. Ons is medegedeeld dat naar verwachting na het zomerreces begin september de Minister een reactie op het gezondheidsraad-rapport aan de VWS-kamercommissie zal verstrekken. Over de inhoud van het gesprek met de minister, en wat wij hebben medegedeeld, volgt later meer, in een tweede deel van deze post. Toegezegd is dat dat meegewogen wordt in de formuleringen van het advies dat dhr Bruins aan de VWS-commissie zal geven.

Groep ME Den Haag: Open schrijven van 73 internationale ME-experts aan de minister van VWS Bruno Bruins
Zojuist deed de Groep ME Den Haag verslag van een bijeenkomst met drie stafleden van minister van VWS dhr. Bruno Bruins op woensdag 11 juli j.l., waarbij de Groep met twee personen vertegenwoordigd was. Daarnaast waren de ME Vereniging, de Stichting ME en Arbeidsongeschiktheid, de ME/cvs Vereniging en de ME/CVS Stichting uitgenodigd, waarvan de laatste drie met een gezamenlijke tekst kwamen.
In dat verslag is sprake van een brief van 73 internationale experts aan Dhr. Bruins en de VWS-commissie. Vanwege de grote lengte van die brief (vooral door alle ondertekeningen) is die ongeschikt voor plaatsing op de wall van de Groep en volgt hij hieronder. Alle experts stemden in met de publicatie van deze brief.

Open schrijven van 73 internationale ME-experts (de originele brief is uiteraard in het Engels)
aan de Nederlandse minister van VWS dhr. Bruno Bruins en de leden van de vaste kamercommissie van VWS:
“In zijn op 19 maart 2018 gepubliceerde rapport verandert de Nederlandse Gezondheidsraad drastisch zijn conclusies vergeleken met die in zijn eerdere rapport uit 2005.
Wat in 2005 werd gerapporteerd, komt niet langer overeen met de internationale wetenschappelijke kennis over Myalgische Encefalomyelitis (ME). Gebaseerd op het gereviewde wetenschappelijke bewijs komt de Gezondheidsraad nu tot de conclusie (zoals de Institute of Medicine, IOM in zijn rapport van 2015, de Centers of Disease Control, CDC en de National Institutes of Health, NIH) dat ME (wat in het rapport ME/CVS wordt genoemd) een ernstige multisysteemziekte is die de activiteiten en levenskwaliteit van de patiënten ernstig beperkt.
De patiënten lijden aan een echte en fysieke ziekte waarvoor op dit moment geen genezing is. Het is geen psychologische of psychosomatische ziekte. Post Exertional Malaise, een verergering van de symptomen na minimale fysieke of mentale inspanning, is kenmerkend voor de ziekte. Er is sterk bewijs van neurologische/autonome verstoringen, immunologische en inflammatoire pathologieën, verstoring van het microbioom, metabole en/of mitochondriële afwijkingen (en meer) bij patiënten.
Patiënten krijgen nu al decennia lang behandelingen voorgeschreven als cognitieve gedragstherapie (CGT) en graded exercise therapie (GET) die gebaseerd zijn op het denkbeeld dat ze lijden aan ‘verkeerde ziekte-percepties’, angst om te bewegen of dat zij gedeconditioneerd zijn. En op het denkbeeld dat ‘in stand houdende’ factoren moeten worden aangepakt om te verbeteren of zelfs te genezen. De afgelopen 20 jaar hebben de op die hypothese (van CGT/GET) gebaseerde behandelingen geen degelijk wetenschappelijk bewijs opgeleverd, zoals de systematische literatuur-review van (het addendum van)de AHRQ en de heranalyse van de PACE-trial, de grootste studie ooit aangaande CGT/GET, hebben aangetoond.
De CDC hebben onlangs de aanbevelingen van CGT en GET van de website verwijderd. Bovendien hebben patiënten internationaal al langer dan 20 jaar continu een verslechtering gerapporteerd na het opvolgen van de raad van hun artsen om hun inspanningsniveaus langzaam te verhogen, gebaseerd op een GET-protocol (dat in Nederland bijna altijd standaard deel uitmaakt van CGT).
Er is internationale overeenstemming dat het subsidiëren van biomedisch onderzoek de enige manier is om tot betere inzichten te komen in de fysiologische mechanismes van deze invaliderende ziekte, en de zo noodzakelijke biomerkers te ontdekken voor de diagnose. Zodat we dichterbij het verstrekken van betere zorg kunnen komen (gebaseerd op het biomedische karakter van de ziekte en de zorgbehoeften van de patiënten) en in de toekomst effectieve behandelingen of een genezing.
Ook is het nodig het verhaal van deze ziekte te veranderen zodat hij niet meer verkeerd gediagnosticeerd of gestigmatiseerd wordt door hem onterecht op gelijke hoogte te stellen met (chronische of onverklaarde) vermoeidheid, of psychosomatische klasseringen (zoals medisch onverklaarde symptomen, somatoforme stoornissen, somatische symptoomstoornis, functioneel somatisch syndroom, neurasthenie, of BDD/BDS (bodily distress disorder/syndrome).
Wij willen de Nederlandse regering op het hart drukken de aanbevelingen van de Nederlandse Gezondheidsraad (en van de IOM, CDC en NIH) op te volgen, wat een belangrijke en lange termijn investering inhoudt in biomedische research. Dat is een absolute prioriteit en de enige manier om de hoognodige vooruitgang te boeken om wat de CDC een ‘verborgen gezondheidscrisis’ noemt te helpen stoppen. Wij verstrekken u zo nodig heel graag meer inzichten die gebaseerd zijn op onze eigen expertise.”
